Everything you need to get started as a participant in the Animation Guild 401(k) Plan. This booklet includes the Enrollment Form, Beneficiary Form and Rollover Form (see below), and all of the investment fund reports.


Individual reports are available for each of MassMutual's investment fund options. Included in the Benefits Booklet (above).


To be used to:

NOTE: MassMutual will charge you a fee to use this form to change your investments. To avoid this fee, make your investment changes on the MassMutual website or by using the toll-free number (see the Quick Info Card below.) This form is included in the Benefits Booklet (above).


Must be completed by all new Plan participants before contributions can be accepted. This form can also be used to change beneficiary designations (for example, in case of marriage, divorce, death of a beneficiary or birth of a child.) This form is included in the Benefits Booklet (above).


To be used to transfer funds INTO the Animation Guild 401(k) Plan from a tax-deferred account (such as another 401k Plan). To transfer funds out of the Animation Guild 401(k) Plan, use the Distribution Form (below). This form is included in the Benefits Booklet (above).


To be used to transfer funds OUT OF the Animation Guild 401(k) Plan, either as a rollover from the TAG Plan into another 401(k) Plan or tax-deferred account, or as a cash distribution. To transfer funds into the Animation Guild 401(k) Plan, use the Rollover Form (above).


To be filed on behalf of the beneficiary(ies) in the event of the death of a Plan participant.


If you initiate a loan against your 401(k) Plan account after June 12, 2005, MassMutual will require that payments be automatically deducted from your bank account. For loans initiated before May 16, 2005, MassMutual will provide new coupons after June 12.

Participants with outstanding loans as of May 16, 2005 should have received a letter from MassMutual, along with an Authorization Agreement for Participant Loan Debit to complete and return if you wish to convert your payment system from coupons to automatic bank account debit.
